About the Role Supportive Bridge Staffing Agency is proud to partner with Freshney Pelham Care Ltd to source a dedicated and compassionate Community Staff Nurse. This exciting opportunity is open to experienced nurses, newly qualified professionals, and final-year students due to complete their qualification imminently. Operating as a community nursing team across North East Lincolnshire, Freshney Pelham Care works closely with Primary Care Networks and local GP practices to provide personalised nursing care, prevent hospital admissions, manage end-of-life care, and support patients in maintaining their independence at home.

Key Responsibilities

• Deliver high-quality, evidence-based nursing care and support to patients, families, and carers in their own homes.

• Manage an unpredictable and sometimes complex caseload day-to-day, demonstrating strong organisational and prioritisation skills.

• Assess patients and carers holistically, implementing, monitoring, and evaluating effective care plans.

• Maintain clear, contemporaneous, and accurate records using IT systems.

• Work collaboratively within a multi-disciplinary team alongside Primary Care Networks, Social Care, Specialist Nursing Teams, and other healthcare professionals.

Person Specification & Requirements

• Qualifications & Registration: Successful completion of a Diploma or Degree in Nursing, with current registration as a Registered Nurse on the NMC Register (plus a full UK driving licence and access to your own vehicle for travel across North East Lincolnshire).

• Skills & Competencies: Effective verbal and written communication skills, strong IT literacy, the ability to make autonomous decisions, and a proven capability to work on your own initiative as well as part of a team.

• Working Pattern: As a 365-day, seven-day community service operating between 8:00 AM and 8:00 PM, flexibility to work on a rota basis—including evenings, weekends, and bank holidays—is essential.

• Compliance: Enhanced Disclosure and Barring Service (DBS) check required alongside valid UK professional registration.
