You will collaborate with families, healthcare professionals, and care home management to create comprehensive care plans, carry out risk assessments, and oversee medication administration while promoting resident independence.
Key Responsibilities
Supervise and lead shifts, delegate tasks effectively, and assist Care Assistants with daily resident support.
Case-manage residents, carry out risk assessments, and coordinate with stakeholders to build tailored care plans.
Safely administer and log medication in compliance with safety and care home regulations.
Requirements
NVQ Level 3 in Health and Social Care (desirable) alongside demonstrated team leadership capabilities.
Well-organized, IT literate, and knowledgeable in care planning and shift allocation procedures.
