Community Staff Nurse - Thame Band 5

Supportive Bridge Staffing & Recruitment is pleased to assist our valued client in recruiting a motivated Band 5 Community Staff Nurse to join their Thame and Princes Risborough community team. Based at Thame Community Hub under Buckinghamshire Healthcare NHS Trust, this 12-month fixed-term/maternity cover position is a part-time role requiring 30 hours per week (including sporadic weekends and late shifts).

In this community-based role, you will work within a multidisciplinary locality team alongside District Nurse Team Leaders, Therapists, and Healthcare Assistants to deliver holistic care to patients in their own homes and residential settings. You will manage complex healthcare needs, execute advanced nursing interventions, support admission avoidance initiatives, and collaborate closely with GPs, social care teams, and specialist clinical services across Buckinghamshire.

Role Description

As a Band 5 Community Staff Nurse, you will deliver autonomous, holistic nursing care to patients with complex conditions in home settings, perform clinical tasks, manage acute deterioration, and facilitate admission avoidance pathways.

Key Responsibilities

• Perform holistic clinical assessments and manage individualized care plans for patients with acute and long-term health needs in community settings.

• Deliver specialized nursing treatments, including complex wound care, compression therapy, negative pressure wound therapy, venepuncture, end-of-life care, and medication administration.

• Identify and manage deteriorating patients, maintain accurate electronic records using systems like RIO, and collaborate with GPs and multidisciplinary teams to prevent avoidable hospital admissions.

Requirements

• Active NMC registration as a Registered Nurse (or student nurse awaiting NMC registration confirmation).

• Post-registration or student placement experience in relevant clinical areas such as Elderly Care, Community Nursing, Palliative Care, or Long-Term Conditions.

• Full UK driving licence with daily access to a personal vehicle for business travel across the Trust's locality.
