Care Team Leader

As a Care Team Leader, you will oversee daily shift operations, coordinate care plans with multidisciplinary professionals, and lead care assistants on the floor. You will play an instrumental role in maintaining a safe, structured, and compassionate environment where residents thrive and remain as independent as possible.

Key Responsibilities

• Lead and supervise shift staff, allocate duties, and ensure the accurate administration and recording of medication.

• Case manage residents, conduct risk assessments, and collaborate with families and healthcare professionals to optimize care plans.

• Work closely with the senior management team to develop care services and maintain a high standard of person-centered care.

Requirements

• NVQ Level 3 in Health and Social Care is preferred, alongside proven experience in leading and supervising care staff.

• Strong organizational skills, digital literacy, and a thorough understanding of care planning processes.

• Excellent leadership abilities with a genuine commitment to delivering compassionate, resident-focused care.
