Salary: £13.50 per hour (£26,325 per annum Full-Time Equivalent)

About the Role

We are acting on behalf of FedBucks Ltd to recruit an enthusiastic and compassionate Social Prescribing Link Worker to join the multidisciplinary team at Phoenix Health Primary Care Network (PCN).

In this role, you will provide personalized, holistic support to individuals with long-term conditions, frailty, end-of-life diagnoses, social isolation, and other complex needs across Buckinghamshire and Oxfordshire. Working collaboratively across network practices, you will empower patients to navigate health and social care services, connect with community-based solutions, and build independence to improve their overall wellbeing and quality of life.

Key Responsibilities

1. Patient Support & Holistic Care Planning

• Provide one-to-one, non-judgmental support to patients, facilitating holistic care planning and coordinating input from various professional services.

• Connect individuals with a wide range of community-based services, voluntary groups, and statutory provisions to promote healthy lifestyles and maintain independence.

• Help reduce avoidable hospital admissions, residential care placements, and routine GP appointments by addressing the wider social, economic, and environmental determinants of health.

2. Risk Management & Safe Signposting

• Identify, assess, and manage risks when working directly with individuals, maintaining a strong awareness of when to safely refer patients back to qualified healthcare practitioners (e.g., specialized mental health needs).

• Provide clear information and advice to patients, families, and carers in a way that inspires trust and confidence.

3. Professional Development & Training

• Complete or maintain enrollment in the NHS England online learning programme for social prescribing.

• Actively undertake or complete appropriate training frameworks set out by the Personalised Care Institute.

Person Specification

Essential Criteria

• Experience: At least 1 year of experience as a Social Prescriber Link Worker, or direct experience working in community development, adult health and social care, learning support, public health, health improvement, or related support roles (including unpaid work).

• Qualifications: NVQ Level 3 (Advanced level or equivalent) or working towards it, with a demonstrable commitment to continuous personal and professional development.

• Skills & Knowledge: A strong understanding of the wider determinants of health and good IT literacy (ability to use word processing, email, and the internet to create simple plans and reports).

• Personal Attributes: Exceptional listening and empathy skills; ability to connect respectfully with people from diverse backgrounds; strong communication abilities; and a firm commitment to reducing health inequalities.

Desirable Criteria

• Knowledge of personalized care approaches, community development models, and local VCSE/community services within the PCN region.

• Prior experience supporting mental health needs or collaborating with the voluntary, community, and social enterprise (VCSE) sector.
