Location: Eyam Surgery (with flexibility to support Bradwell Surgery)
Salary: £24,544 per annum (pro rata for part-time hours)

As a Care Navigator with our supportive bridge staffing agency, you will play an essential role in keeping primary care running smoothly every single day. Based at Eyam Surgery with travel requirements between sites, you will serve as the welcoming, professional first point of contact for patients and visitors—guiding individuals to the most appropriate healthcare services and ensuring they receive a positive, compassionate experience from the moment they connect with the practice.
Welcome patients and visitors professionally, acting as the primary point of contact via phone and face-to-face interactions.
Expertly signpost patients to the most appropriate services, including community pharmacy, social prescribing, physiotherapy, and other local care pathways.
Coordinate patient appointments using practice clinical systems and process repeat prescription requests accurately.
Communicate test results to patients and handle incoming emails and phone messages in a timely, secure manner.
Manage daily operational tasks, including opening and closing surgery premises, maintaining tidy reception environments, and supporting clinical room preparation.
Provide flexible cover for planned and unplanned staff absences across both the Eyam and Bradwell surgery sites.
Excellent communication, interpersonal, and customer service skills.
Strong organizational abilities with the capacity to multitask calmly under pressure in a busy environment.
Good IT literacy and confidence using digital systems and clinical software.
