About the Role
We are acting on behalf of ELM Alliance Ltd—the local GP Federation that took responsibility for Ravenscar Surgery in October 2021—to recruit an experienced Advanced Nurse Practitioner (ANP).
Based in purpose-built premises at Redcar Primary Care Hospital, Ravenscar Surgery is a small, dedicated practice looking after approximately 3,800 patients as part of the Redcar PCN. Co-located with the Urgent Treatment Centre and GP beds, the practice offers a supportive leadership team backed by the ELM Alliance Federation for HR, governance, and clinical mentorship.
In this pivotal role, you will undertake normal ANP duties within primary care, managing a caseload and dealing with a wide range of undifferentiated and undiagnosed health needs. You will provide autonomous face-to-face appointments, telephone consultations, home visits, and care home ward rounds, while contributing to the clinical leadership and management of patient services.
Key Responsibilities
1. Clinical Care & Patient Management
Use advanced clinical nursing practice to assess health needs, formulate appropriate care plans, and deliver high-quality care and advice for registered and temporary patients.
Manage patients with complex and long-term conditions, providing primary prevention, screening services, and chronic disease management.
Prescribe medication appropriate to patient needs as an Independent Prescriber, or refer patients to community pharmacies for minor ailment services.
Handle on-call duties, emergency care, path lab results, investigation reports, and hospital correspondence.
2. Leadership & Practice Development
Provide leadership, support, and mentorship to the Practice Nurse, Health Care Assistants (HCAs), and the wider nursing team.
Practice within the RCN 4 pillars (Advanced Clinical Practice, Leadership, Education/Learning, and Research/Development) and contribute to clinical governance and audit.
Support practice and PCN delivery initiatives, including vaccination programmes and new working practices.
Person Specification
Essential Criteria
Qualifications & Registration: Fully qualified ANP meeting RCN standards, educated to Master's level (180 Level 7 points / Scottish level 11), with an active, clean NMC registration, Independent Prescribing qualification, and physical assessment (adult and child) certification.
Experience: Significant experience working as an ANP within a General Practice / primary care setting, managing complex/long-term conditions, and working autonomously with robust risk management skills.
Competencies: Excellent communication, problem-solving, and time-management skills; adherence to confidentiality; a full, clean UK driving licence; and the flexibility to work outside core office hours.
Vetting: Ability to successfully complete an enhanced Disclosure and Barring Service (DBS) check.
Desirable Criteria
Previous experience using the TPP SystmOne clinical system.
Mentorship qualifications or experience in change management and quality improvement.
