Base Location: The Penn Surgery, Elm Road, Penn, High Wycombe, Buckinghamshire, HP10 8LQ (with travel across ARC Bucks PCN member practices)

Salary: From £55,000 per annum (dependent on experience)

Contract Type: Permanent

Working Pattern: Full-time, 37.5 hours per week, Monday to Friday (Part-time options considered)

About the Role

We are acting on behalf of FedBucks Ltd to recruit a dedicated and forward-thinking Clinical Pharmacist to join the expanded in-house pharmacy team at ARC Bucks Primary Care Network (PCN).

This is a unique opportunity to help shape a growing, innovative service that empowers pharmacists to operate at the top of their professional competence. Working closely alongside GPs, nurses, pharmacy technicians, and multidisciplinary colleagues across four South Buckinghamshire practices serving around 50,000 patients, you will deliver expert medicines optimisation, lead long-term condition management, and improve clinical outcomes across the network.

Key Responsibilities

1. Patient-Facing Clinical Care

• Conduct structured medication reviews (SMRs) for patients with complex needs and polypharmacy.

• Manage long-term condition clinics (including asthma, COPD, and hypertension) and facilitate group consultations.

• Provide expert medicines optimisation reviews for care home residents, housebound patients, and vulnerable populations following hospital discharges.

• Offer expert, evidence-based medicines advice to patients, carers, and clinical colleagues.

2. Medicines Optimisation & Safety

• Enhance prescribing quality, safety, and cost-effectiveness across the PCN using clinical searches and population health tools.

• Implement MHRA safety alerts, national prescribing guidelines, and local quality improvement initiatives.

• Reduce medicine waste and boost patient adherence through proactive, person-centred consultations.

3. Quality Improvement & Collaboration

• Participate in clinical audits, QOF, and IIF contractual priority delivery.

• Build strong working relationships with community pharmacies, secondary care, and system partners to foster a culture of continuous learning and shared improvement.

Person Specification

Essential Criteria

• Registration & Qualifications: Registered Pharmacist with the General Pharmaceutical Council (GPhC), holding current UK professional registration, alongside an Independent Prescriber qualification (or a willingness to undertake training).

• Experience: Prior experience working within primary care, community pharmacy, or hospital pharmacy settings.

• Skills & Competencies: Excellent clinical knowledge, strong communication and consultation skills, and proven experience utilizing the EMIS GP Clinical System.

• Attributes: Ability to work independently while contributing effectively as part of a collaborative multidisciplinary team.

Desirable Criteria

• Previous experience conducting Structured Medication Reviews (SMRs) or working within a PCN/General Practice.

• Hands-on experience in long-term condition management, clinical audits, quality improvement, teaching, or mentoring.
