Location: Multi-site across Leatherhead Primary Care Network practices (including Eastwick Park Medical Practice, Fairfield Medical Centre, Ashlea Medical Practice, and The Molebridge Practice)
Salary: £30,908 per annum (FTE)
Contract Type: Permanent
Working Pattern: Part-time (22.5 to 30 hours per week)

We are acting on behalf of Dorking Healthcare Ltd (DHC)—an innovative NHS provider organisation serving Surrey—to recruit a compassionate and proactive Social Prescriber for the Leatherhead Primary Care Network (PCN).
In this vital role, you will bridge clinical care and community support by helping individuals address non-medical factors affecting their health and wellbeing, such as social isolation, housing, employment, financial concerns, and lifestyle issues. Working across four GP practices and local community sites, you will empower patients to take greater control of their health by connecting them with vital local services and voluntary networks.
1. Patient-Centred Support & Wellbeing Plans
Receive referrals from GPs, healthcare professionals, and partner agencies, managing and coordinating appointments for patients with wellbeing needs.
Conduct sensitive, person-centred conversations and holistic assessments to help individuals identify personal health goals and independence milestones.
Develop personalised wellbeing plans, offering ongoing support, follow-up reviews, and guidance to achieve agreed outcomes.
2. Community Connection & Multidisciplinary Working
Signpost, refer, and connect patients to appropriate community resources, voluntary sector organisations, and local support networks.
Build and maintain strong, collaborative relationships with healthcare professionals, community partners, and outreach providers.
Attend multidisciplinary team (MDT) meetings, identify safeguarding concerns, and maintain accurate records on clinical IT systems in line with data protection standards.
Essential Criteria
Education & Skills: Good standard of education with strong literacy, numeracy, IT, administrative, and record-keeping skills.
Experience: Experience working with the general public in a similar customer-facing role, backed by practical experience within healthcare, social care, community development, housing, welfare, or voluntary sectors.
Personal Qualities: Polite, confident, caring, sensitive, and empathetic, with a strong ability to remain calm under pressure, build quick rapport, and work both independently and collaboratively.
Requirements: Willingness to travel across multiple primary care sites and locations, and ability to successfully complete an enhanced Disclosure and Barring Service (DBS) check. (Note: This role is not eligible for visa sponsorship; applicants must have the existing right to work in the UK.)
Desirable Criteria
Previous experience working within an NHS / Primary Care General Practice setting.
Familiarity with clinical or case-management IT systems.
