About the Role
We are acting on behalf of DGS Health LTD—a clinically led GP Federation representing 100% of local practices across Dartford, Gravesham, and Swanley—to recruit an experienced and dedicated Frailty Nurse to join their dynamic community frailty team.
In this vital role, you will meet the primary care needs of vulnerable older adults living with frailty in the community. Working collaboratively within a multidisciplinary team, your work will directly reduce unnecessary hospital admissions and ambulance call-outs while improving health and social care support for patients and their carers across North Kent.
Key Responsibilities
1. Clinical Assessment & Care Management
Assess, plan, implement, and evaluate specialist treatment and care through personalised support plans for an allocated caseload.
Review medical issues, complete comprehensive medication reviews, conduct dementia screenings, and develop robust Treatment Escalation Plans (TEPs) and Advanced Care Plans.
Support care home residents, promote patient independence, and provide specialist advice regarding the clinical management of frailty.
2. Multidisciplinary Collaboration & Admission Avoidance
Partner with community health, acute trusts, ambulance services, social care, voluntary organizations, and end-of-life care providers to deliver seamless, joined-up care.
Participate in multidisciplinary team (MDT) meetings and liaise with local organisations to support mental health, wellbeing, and crisis avoidance at home.
Support carers by identifying signs of deterioration and connecting families with local support networks.
Person Specification
Essential Criteria
Qualifications & Registration: Registered practitioner to degree or diploma level, supplemented by specialist training/short courses (or extensive experience), including the completion of a history-taking and physical examination course. Active UK professional registration (NMC/HCPC) is mandatory.
Experience: Proven experience at Practitioner Band 6 level, with recent community experience working with older adults living with frailty and managing clinical change.
Competencies: Strong clinical reasoning, risk assessment, leadership, and communication skills, with the ability to work autonomously and prioritize workloads effectively.
Requirements: Clean UK driving licence with access to a vehicle (essential for community travel across Dartford, Gravesham, and Swanley), and the ability to successfully pass an enhanced Disclosure and Barring Service (DBS) check.
