Base Location: Penn Surgery, Elm Road, Penn, High Wycombe, Buckinghamshire, HP10 8LQ (with travel across ARC Bucks PCN practices)
Salary: From £28,000 per annum (dependent on experience)
Contract Type: Permanent
Working Pattern: Full-time, 37.5 hours per week, Monday to Friday (part-time options will also be considered)

We are acting on behalf of FedBucks Ltd to recruit an enthusiastic and compassionate Health and Wellbeing Coach to join the ARC Bucks Primary Care Network (PCN) team.
This is much more than simply giving lifestyle advice. You will empower patients to make lasting, positive changes that improve their health, confidence, and quality of life. Working collaboratively alongside GPs, nurses, pharmacists, social prescribing link workers, and care coordinators, you will help patients take control of their health, set meaningful goals, and navigate long-term condition prevention and management.
1. Personalised Health Coaching
Provide one-to-one health coaching utilizing personalized care and evidence-based behavior change techniques.
Support patients to identify what matters most to them, build resilience, and establish achievable health and wellbeing goals.
Empower patients to become active participants in their own healthcare through ongoing, motivational coaching sessions.
2. Long-Term Condition Prevention & Management
Support patients living with—or at risk of—long-term conditions such as diabetes, hypertension, cardiovascular disease, and obesity.
Deliver targeted lifestyle interventions and support the rollout of weight management and healthy lifestyle programs.
Work closely with clinical teams to promote early intervention and proactive preventative care.
3. Community Working & Quality Improvement
Connect patients with local community activities, peer support groups, and voluntary sector services alongside Social Prescribing Link Workers.
Deliver health education sessions, group programs, and community events.
Maintain accurate clinical records using GP clinical systems (such as EMIS) and collect outcome data to contribute to QOF, IIF, and wider quality improvement programs.
Essential Criteria
Experience: Proven background supporting people to improve their health and wellbeing through coaching, education, or behavior change.
Communication & Interpersonal Skills: Excellent communication and motivational interviewing skills with a warm, compassionate, and person-centered approach.
Attributes & Abilities: Strong organizational and time-management skills, ability to build rapport easily, and capable of working both independently and within a multidisciplinary team.
Digital Literacy: Good IT and digital skills to maintain accurate clinical documentation.
Qualifications: Educated to at least GCSE standard (Note: Support/willingness to undertake accredited Health Coaching training is provided).
Desirable Criteria
Experience in social prescribing or delivering group education/lifestyle programs.
Familiarity with the EMIS GP clinical system.
Knowledge of local voluntary and community services, or an understanding of health inequalities and personalized care.
