About the Role
We are acting on behalf of Teesside Mind Ltd—a vibrant local mental health charity with a 30-year history of community impact—to recruit a dedicated Mental Health Recovery Worker.
Working as part of the Foundation and Recovery Service under Impact on Teesside, you will support individuals with complex mental health and social care needs (such as housing, debt, and barriers to support). By providing assertive outreach and proactive brokerage, your role is pivotal in helping vulnerable individuals build independence, maintain personal control, and avoid unnecessary crisis care or A&E admissions.
Key Responsibilities
1. Assertive Outreach & Recovery Support
Deliver an assertive, community-based recovery service using a person-centred and enabling approach.
Contribute to triage, screening, and robust risk assessments to identify suitability, eligibility, and care pathways.
Manage complex casework and collaborate directly with service users to implement structured support plans and outcome measurement tools.
2. Partnership Working & Coordination
Liaise closely with external partner agencies—including GPs, secondary mental health teams, the criminal justice system, and voluntary organizations—to ensure coordinated, holistic care.
Support service users in maintaining engagement with appropriate services, working towards positive, planned endings and transitions.
Maintain accurate case records, produce case studies and evidence for reports, and promote the service actively within the local community.
Person Specification
Essential Criteria
Qualifications: Level 3 Health and Social Care qualification or equivalent.
Experience: Proven experience working with people experiencing mental health needs and vulnerable adults with complex needs within a community setting, successfully delivering planned interventions and managing support plans.
Competencies: Strong skills in person-centred assessment, risk management, collaborative partnership working, complex case coordination, and excellent IT/administrative proficiency.
Requirements: Caring, compassionate, and recovery-focused approach; ability to work independently using own initiative; full UK driving licence and access to a vehicle to travel across Teesside for work purposes. Enhanced DBS clearance required.
Desirable Criteria
Graduate-level qualification in a relevant field.
