Base Location: Penn Surgery, Elm Road, Penn, High Wycombe, Buckinghamshire, HP10 8LQ (with travel across ARC Bucks PCN practices)
Salary:
Post 1 (Part-Time): 18.8 hours per week – From £13,000 per annum (pro rata, dependent on experience)
Post 2 (Full-Time): 37.5 hours per week – From £27,675 per annum (dependent on experience)

We are acting on behalf of FedBucks Ltd to recruit compassionate, organized, and proactive Care Coordinators to join the ARC Bucks Primary Care Network (PCN) team.
This is much more than an administrative role. You will bridge the gap between patients, healthcare professionals, and diverse services, ensuring patients do not get lost in the system. Working alongside GPs, nurses, pharmacists, social prescribing link workers, and health and wellbeing coaches, you will combine patient-facing care navigation with practical PCN administration to make healthcare feel more joined-up and accessible.
1. Personalised Care & Patient Support
Use population health intelligence to proactively identify cohorts of patients who would benefit from personalized care.
Act as a key point of contact, advocate, and information resource for patients, families, and carers.
Support patients in shared decision-making, helping them build knowledge, skills, and confidence to manage their health and wellbeing.
Answer queries, arrange appointments, and signappropriate patients to self-management education and peer support.
2. Care Co-ordination & Navigation
Coordinate and navigate care across health, social care, and community services, maintaining regular communication with care homes and external teams.
Undertake community, home, or care home visits where required and appropriate.
Facilitate referrals to appropriate multidisciplinary team members and maintain accurate caseload records.
3. PCN & MDT Administration
Support the organization and delivery of multidisciplinary team (MDT) meetings, including diary management and report generation.
Liaise with colleagues across ARC Bucks PCN member practices to ensure consistent service delivery.
Provide administrative support to Senior PCN Clinicians and Health & Wellbeing Coaches, including clinic scheduling and tracking progress against PCN targets.
Essential Criteria
Experience: Proven background in a care coordination, healthcare, social care, administrative, or similarly patient-focused role.
Communication & Interpersonal Skills: Excellent communication abilities with a warm, compassionate, and person-centered approach.
Organizational Skills: Strong ability to manage competing priorities, problem-solve independently, and follow actions through to completion.
IT & Digital Literacy: Good IT skills with the ability to maintain accurate, timely records while respecting data protection and confidentiality.
Qualifications: Educated to at least GCSE standard.
Desirable Criteria
Experience working within primary care, community healthcare, or the voluntary sector.
Familiarity with the EMIS GP clinical system.
Prior knowledge of local health, social care, and community services, or an understanding of health inequalities.
