Supportive Bridge Staffing Agency is proud to partner with the British Red Cross to recruit a compassionate, permanent Support Worker for their Assisted Discharge team across Royal Bournemouth and Poole hospitals in Dorset.
In this vital part-time role, you will act as a lifeline for vulnerable individuals returning home after a hospital stay, helping them recover safely, regain their independence, and feel supported. No formal care experience is necessary—if you are kind, reliable, and patient, you have everything it takes to succeed!
Assisted Discharge & Home Support: Provide essential practical and emotional support to adults leaving hospital, helping them transition safely back into their own homes.
Welfare & Daily Living: Conduct welfare calls, home visits, and assist with practical tasks such as shopping and following personalised support plans.
Clinical & Community Liaison: Respond effectively to referrals from hospital clinicians and community health professionals to ensure seamless care.
Relationship Building: Provide a friendly, reassuring face and build meaningful connections that empower service users during a vulnerable time.
Personal Qualities: A kind, calm, and confident approach with exceptional people skills and a passion for helping others.
Core Requirements: Full UK Manual Driving Licence held for a minimum of 2 years, with a willingness to drive service vehicles.
Skills: Basic IT literacy, strong communication skills, and the ability to work both independently and collaboratively within a team.
Availability: Flexibility to work on a 7-day rotating schedule, including weekends and Bank Holidays.
Competitive Pay: £24,479 pro-rata salary (£13.45 per hour).
