About the Role
We are acting on behalf of FedBucks Ltd to recruit a compassionate and motivated Social Prescribing Link Worker to join the ARC Bucks Primary Care Network (PCN) multidisciplinary team.
This role goes far beyond simple service referrals. You will help people regain their confidence, improve their overall wellbeing, and take control of their health. Working with individuals whose health is impacted by wider social, emotional, or practical issues—such as loneliness, financial pressures, housing concerns, or caring responsibilities—you will connect them with community support to live happier, more independent lives.
Key Responsibilities
1. Patient Support & Personalised Care
Provide personalised one-to-one support through meaningful social prescribing conversations and motivational interviewing techniques.
Develop collaborative care and support plans tailored to each individual's unique goals and needs.
Support patients to build resilience, independence, and long-term self-management strategies.
2. Community Connections & Partnership
Connect individuals with a wide network of voluntary, community, and statutory services.
Build and maintain robust relationships with local organizations, community groups, and wellbeing activities, identifying gaps to help foster new opportunities.
Work collaboratively as an integral member of the PCN integrated neighborhood team, alongside GPs, nurses, pharmacists, care coordinators, and health and wellbeing coaches.
3. Reducing Health Inequalities & Governance
Support vulnerable groups experiencing social isolation and the wider determinants of health to improve accessibility.
Maintain accurate, timely clinical records using GP clinical systems (such as EMIS) and monitor outcomes to demonstrate impact.
Participate in regular supervision, training, and continuous service development initiatives.
Person Specification
Essential Criteria
Experience: Proven experience supporting people within health, social care, community, or voluntary sector settings, including prior experience with social prescribing.
Interpersonal Skills: Excellent communication and relationship-building abilities, underpinned by a compassionate, person-centered approach.
Attributes & Abilities: Strong organizational and time-management skills, with the capability to motivate and empower individuals while working both independently and in a multidisciplinary team.
Digital Literacy: Good IT skills to maintain clinical documentation accurately.
Qualifications: Educated to at least GCSE standard.
Desirable Criteria
In-depth knowledge of local voluntary and community services in Buckinghamshire.
Practical experience using the EMIS GP clinical system.
A strong understanding of personalized care frameworks, health inequalities, and behavior change techniques (e.g., motivational interviewing).
