Role: Support Worker - Discharge to Assess
Location: South Ayrshire (with travel around the area)
Salary: £24,479 per annum based on 35 hours per week, pro-rata for roles under 35 hours (£13.45 per hour)
Hours: 18, 24 & 35 hours available (Permanent)

Are you passionate about helping people live independently in their own homes? As a staffing partner, Supportive Bridge Staffing Agency is recruiting compassionate and driven individuals for this Support Worker position on behalf of our client.
In this vital role, you will be at the heart of a regulated service, responding to referrals from health professionals to provide practical and emotional support to vulnerable adults. Your work will help prevent hospital re-admissions, promote wellbeing, and enable individuals to regain confidence and independence in South Ayrshire.
Client Assessments: Visit service users in their homes to evaluate needs and establish tailored support plans.
Daily Living Assistance: Support individuals with everyday tasks, including personal care (bathing, dressing) and meal preparation.
Multi-Agency Collaboration: Liaise closely with health, housing, and social care professionals to guarantee consistent care delivery.
Welfare & Independence: Carry out regular welfare checks and empower service users to rebuild their long-term independence.
Care Experience: Prior experience supporting people with physical, sensory, or mental health needs.
Communication Skills: Strong interpersonal abilities with the capacity to work both autonomously and as part of a dedicated team.
Sector Knowledge: Familiarity with NHS and social care services, alongside how to access them effectively.
Flexibility & Requirements: A flexible approach to working hours (including evenings and weekends). Note: Successful candidates must register with the Scottish Social Security Council (SSSC) within 3 months of starting. Unfortunately, sponsorship is not available for this role.
Leave: 36 days annual leave (including bank holidays) with the option to buy 5 extra days.
