Supportive Bridge Staffing & Recruitment is pleased to assist our valued client in recruiting an experienced Home Based Treatment Mental Health Practitioner (Band 6) to join their urgent care team in Leigh, UK. Based at Atherleigh Park Hospital within an established NHS Foundation Trust, this permanent post offers an excellent opportunity for mental health professionals to deliver intensive community crisis intervention as an alternative to inpatient admission.

In this full-time position working 37.5 hours per week across a 24-hour shift pattern (including night shifts and rotational weekend work), you will collaboratively agree care plans with service users and their carers. You will conduct urgent care assessments, manage clinical risks, facilitate post-discharge follow-ups, deliver home treatment interventions, and gatekeep hospital admissions within a dynamic multidisciplinary environment.

Role Description

As a Band 6 Home Based Treatment Mental Health Practitioner, you will deliver urgent mental health assessments, implement community crisis care plans, gatekeep inpatient admissions, and manage clinical risks across 24-hour rotational shifts.

Key Responsibilities

  • Assess, plan, and deliver home treatment interventions for individuals experiencing severe mental health crises to prevent avoidable hospital admissions.

  • Conduct robust risk assessments, manage clinical risk, and adhere to safeguarding protocols and the Mental Health Act (1983).

  • Work flexibly across 24-hour rotational shift patterns, collaborating with multidisciplinary urgent care teams and external agencies to ensure continuous patient safety and care flow.

Requirements

  • Active professional registration with the NMC (Sub Part 1 Mental Health) or an equivalent professional regulatory body.

  • Demonstrated expertise in mental health risk assessment and clinical risk management, ideally within an urgent care or home treatment setting.

  • Clear understanding of community and inpatient care pathways, mental health legislation, and safeguarding principles for vulnerable adults and children.
