Based at the Heddfan Unit within Wrexham Maelor Hospital, this 3-year full-time secondment cover position offers an exceptional leadership opportunity within the largest health board in Wales.
In this full-time role working 37.5 hours per week, you will work alongside the Service Lead to oversee the East Locality Home Treatment Team. You will lead operational service delivery aimed at supporting individuals in their own homes, preventing unnecessary hospital admissions, and facilitating timely inpatient discharges. You will balance bed-flow management with community support priorities, drive quality improvement (QI) initiatives, and manage team performance targets within a multidisciplinary environment.
Role Description
As a Band 7 Home Treatment Team Manager, you will manage operational performance, lead multidisciplinary staff, oversee crisis resolution and admission avoidance strategies, and drive service improvement projects across the East Locality.
Key Responsibilities
Provide operational management and clinical leadership for the Home Treatment Team to ensure efficient admission avoidance and early discharge pathways.
Balance unit bed-flow demands with community care delivery while ensuring performance targets and quality metrics are met.
Implement Quality Improvement (QI) strategies, foster multi-disciplinary team collaboration, and maintain robust risk formulation and clinical safety standards.
Requirements
Relevant professional qualification in health, social care, or equivalent senior operational management experience within mental health.
Proven track record of team management and leadership within urgent, acute, or community mental health settings.
Strong knowledge of mental health issues, risk formulation, quality improvement frameworks, and service performance management.
Benefits
Calculated hourly rate of £25.71 to £29.42 per hour (based on £50,129 - £57,365 per annum across a standard 37.5-hour work week).
