We are seeking caring, patient, and motivated individuals who are enthusiastic, possess great people skills, and maintain a proactive approach to supporting clients with Learning Disabilities, Autism, Mental Health, and complex needs living in their own homes.

What You Will Be Doing (Outreach Support)

As part of your day-to-day work, you will be empowering clients to live as independently as possible through:

• Encouraging: Empowering people to play active roles in their communities and foster independence.

• Supporting: Assisting with activities of daily living, including social activities, personal care, routines, meal planning and preparation, and budgeting.

• Communicating: Communicating effectively using each individual's preferred methods.

• Engaging: Promoting and motivating the people we support to enhance their overall quality of life.

• Specialising: Supporting individuals who may benefit from positive behavioural support approaches.

• Recording: Keeping accurate records and reporting changes in health or circumstances to management.

• Team Player: Working collaboratively as part of a team while maintaining the ability to use your own initiative.
