Supportive Bridge Staffing Agency is proud to partner with Bradford Council to recruit a dedicated and compassionate Reablement Assistant (Days) within the Adult Social Care & Health department.
Based at Dove Court Extra Care Setting—home to 45 properties supporting adults with varying levels of need—you will work collaboratively within a supportive team to help residents live happy, healthy, and independent lives. (Please note: We are unable to offer sponsorship for this role).
As a Reablement Assistant, you will provide person-centered support to a diverse service user group, predominantly adults aged 35 and over with varying disabilities, mental health needs, dementia, and complex requirements. Key responsibilities include:
Independence & Daily Living: Supporting service users with individual needs, daily living skills, and personal care while encouraging maximum independence.
Medication & Reporting: Administering medication in strict accordance with policy and completing clear, concise daily reports and support plans.
Visitor Support: Greeting, assisting, and directing visitors within the extra care setting professionally and warmly.
Team Collaboration: Working flexibly as part of an integrated multi-disciplinary team to ensure a safe, caring, and vibrant community environment.
We are looking for a flexible, multifaceted, and caring individual who brings:
Qualifications Requirement: A QAF Diploma (NVQ / QCF) at minimum Level 2 in Health & Social Care.
Proven experience or a strong passion for supporting adults with complex needs, learning disabilities, dementia, or mental health conditions.
Excellent interpersonal, communication, and organizational skills.
A positive, person-centered approach dedicated to promoting dignity and independence.
