Supportive Bridge Staffing Agency is proud to partner with New Vision Bradford—a powerful partnership delivering adult drug and alcohol services across the Bradford and Craven District.
We are looking for a passionate, driven, and compassionate Outreach Recovery Coordinator to join our Treatment Team in this newly created role. You will deliver high-quality substance misuse treatment directly to service users within their own homes or residential care settings, supporting them from initial point of entry through to the completion of their recovery journey.
About the Role
As an Outreach Recovery Coordinator, you will act as the main care coordinator for an assigned group of service users requiring home interventions. Key responsibilities include:
Outreach Interventions: Delivering dedicated substance misuse support, screening, and interventions directly in service users' homes or residential care settings.
Care Coordination: Conducting comprehensive assessments, asset mapping, recovery planning, and regular reviews of care.
Multi-Disciplinary Collaboration: Working closely within a dynamic team across New Vision Bradford (The Bridge Project, Create Strength Group, Project 6, and Waythrough) to reduce risk-taking behaviors and support long-term recovery.
Flexible Working & Travel: Managing an outreach caseload, requiring a flexible approach to working hours, a valid driving licence, access to a vehicle for work purposes, and business insurance.
About You
We are looking for a dedicated and engaging professional who brings:
Experience Requirement: At least 2 years of full-time experience working with people with alcohol and/or drug problems and/or complex/multiple needs within the last 4 years.
Proven experience in case management and delivering care coordination in drug and alcohol services or a directly related field.
Excellent organisational, communication, and time-management skills with a collaborative, "can-do" approach.
A strong commitment to supporting vulnerable individuals and a willingness to undergo an enhanced DBS check.
