About the Role Supportive Bridge Staffing Agency is proud to partner with K&W Healthcare Ltd—a GP-led federation representing 24 practices across the Brent localities of Kingsbury, Willesden, and Wembley—to source an experienced Clinical Pharmacist with an Independent Prescribing qualification. Working across multiple member practices serving over 300,000 patients, you will provide primary support for prescriptions and medicines management, helping to reduce GP workloads, optimize clinical care, and address public and social care needs.
Key Responsibilities
Conduct patient-facing clinical medication reviews, manage minor ailment triage, and run dedicated clinics to improve medication adherence.
Streamline prescription systems by handling acute requests, reviewing repeat prescriptions, and reconciling medicines following hospital discharge or transfers of care.
Undertake targeted medication reviews for high-risk patient groups (such as the frail elderly, polypharmacy cases, and those with renal or hepatic impairment).
Assist with Quality and Outcomes Framework (QOF) targets by supporting the management of long-term conditions including asthma, COPD, diabetes, hypertension, and heart failure.
Administer influenza and travel vaccines, support substance misuse pathways, and make clinical recommendations to general practitioners and senior staff.
Person Specification & Requirements
Qualifications & Registration: Essential Independent Prescribing (IP) qualification, current UK professional registration, and at least 2 years of prescribing experience.
Experience: Previous clinical experience within a Primary Care or General Practice setting is highly desirable.
Core Skills: Strong clinical decision-making capabilities, excellent patient communication skills, and the ability to work effectively within a multidisciplinary primary care team.
Compliance: Enhanced Disclosure and Barring Service (DBS) check required.
