Salary: Agenda for Change Band 7 (£49,387 to £56,515 per year)

Location: Turton PCN (Operating across multiple practice sites including Edgworth Medical Practice, Bolton, BL7 0DU)

Contract Type: Permanent / Full-Time

About the Role

We are looking for a dedicated and ambitious PCN Practice-Based Clinical Pharmacist to join the well-established and friendly pharmacy team at Turton Primary Care Network, operated via Edgworth Medical Practice Ltd. Across our 6 member practices serving approximately 45,000 patients, you will work as part of a multidisciplinary team and join an existing group of 7 clinical pharmacists. You will play a pivotal, patient-facing role managing chronic diseases, conducting structured medication reviews (SMRs), optimizing medicines, and leading on quality improvement.

Key Responsibilities

• Undertake clinical medication reviews and proactively manage patients with complex polypharmacy and chronic disease conditions.

• Handle medicines-related activities including SMRs, QOF work, medicines optimisation, and CQC audits.

• Provide primary support to general practice staff regarding prescription queries, repeat prescription systems, acute prescription requests, and medicines reconciliation on transfer of care.

• Act as clinical lead on medicines optimisation, quality improvement, and enhanced services, integrating primary care with community and hospital pharmacy networks.

• Work collaboratively within a supportive organizational culture, with mentorship and development overseen by the PCN Manager, Clinical Director, and Lead Pharmacist (including support to train as a non-medical prescriber if not already achieved).

Candidate Requirements

• Qualifications & Registration: Must hold a Master’s Degree in Pharmacy (or equivalent), current GPhC registration, specialist knowledge through a postgraduate diploma/qualification, and evidence of recent CPD. Independent Prescriber status is preferred, or a commitment to undertake the course.

• Experience: At least 2 years of post-registration experience within a hospital, community, or general practice setting, alongside experience working with multidisciplinary teams and conducting clinical audits.

• Travel: Must hold a valid UK driving license and have access to a vehicle for travel across PCN sites.
