Salary: £38,114.53 - £44,469.24 Per Annum + Outer London Weighting (£2,250.61) (Competitive with NHS Agenda for Change pay bands)

Supportive Bridge Staffing Agency is proud to partner with Change Grow Live (CGL) to recruit a compassionate, clinically robust, and dedicated permanent Assertive Outreach Nurse to join their multidisciplinary team in Barnet.
In this vital and varied role, you will improve choice and control for individuals who are rough sleeping, homeless, or at risk of homelessness—reaching out to people who find it difficult to engage with traditional services. Working flexibly across the community, you will provide specialist clinical interventions, physical and psycho-social assessments, wound care, and blood-borne virus (BBV) testing and vaccinations to help individuals transform the direction of their lives.
Clinical Assessments & Care: Carry out physical and psycho-social assessments for the homeless population, delivering evidence-based clinical and therapeutic treatments.
BBV Screening & Vaccination: Provide dry blood spot testing for blood-borne viruses (Hepatitis B, C, and HIV), manage integrated care pathways, and administer Hepatitis A/B vaccinations.
Harm Reduction & Wound Care: Offer wound care assessments and basic dressings, and contribute to the CGL Naloxone Strategy by training staff and service users in its use.
Partnership & Advocacy: Act as an advocate for people experiencing homelessness, working closely with community teams and voluntary organisations to ensure seamless care delivery.
Qualifications & Registration: First-level registered nurse with a current NMC PIN and recent relevant post-qualification experience.
Clinical Knowledge: Sound knowledge of harm reduction, health promotion interventions, and assessment/recovery planning (experience in drug and alcohol treatment is desirable).
Core Competencies: Professional, empathetic, and non-judgmental attitude, with strong communication skills and a commitment to professional integrity.
Requirements: Supportive team player. Must hold the legal Right to Work in the UK (note: this role is subject to an enhanced DBS check).
