Role: Senior Practitioner - Discharge to Assess
Location: South Ayrshire
Salary: £25,541 to £27,200 per annum
Hours: 35 hours per week (Permanent)

In this role, you will be instrumental in delivering high-quality, person-centred care within a regulated service, guiding individuals on their journey from hospital to home. Working closely with the Service Manager, you will ensure vulnerable adults receive compassionate care while mentoring and developing support staff to succeed in their roles.
Coordination & Collaboration: Manage referrals and liaise closely with hospitals, GP surgeries, and multi-disciplinary community teams.
Team Leadership & Mentoring: Guide, support, and mentor staff, students, and volunteers to ensure exceptional care delivery.
Safety & Risk Management: Conduct thorough risk assessments and welfare checks to guarantee safe living conditions.
Operational Continuity: Manage rotas, handle out-of-hours queries, and maintain seamless service operations.
Qualifications: SVQ Level 3 in Health and Social Care and a PDA in Supervision (or equivalent). Note: Successful candidates must register with the Scottish Social Security Council (SSSC) within 3 months of starting.
Driving Requirement: Full UK driving licence and access to a vehicle (business mileage paid).
Leadership & Communication: Strong communication skills with a proven ability to lead, motivate, and mentor others.
Experience: Background in person-centred planning and multi-disciplinary working. Please note: Unfortunately, we are unable to offer visa sponsorship for this role.
