Supportive Bridge Staffing & Recruitment is pleased to assist our valued client in recruiting an experienced Band 6 Mental Health Nurse or Allied Health Professional. Based at the Gleadless and Heeley Neighbourhood Mental Health Centre (Terry Wright Community Hall/Newfield Green Clinic) under Sheffield Health Partnership University NHS Foundation Trust, this 6-month fixed-term/secondment position is a full-time role requiring 37.5 hours per week (working 8am–8pm shifts across 7 days on a rota basis).

In this urgent and crisis care role within one of the UK’s pilot Neighbourhood Mental Health Centre models, you will work in an integrated multidisciplinary Home Treatment Team. You will deliver rapid crisis assessments, coordinate care pathways with primary, secondary, and VCSE partners, formulate crisis plans, support on-site hospitality beds, and facilitate admission avoidance and early supported discharge for adults aged 18 and over in Sheffield.

Role Description

As a Band 6 Mental Health Nurse or AHP, you will triage and conduct comprehensive crisis assessments, devise intervention plans, manage clinical risk, deliver home treatment care, and provide clinical supervision to junior staff.

Key Responsibilities

• Perform comprehensive health, social care, and risk assessments for individuals presenting in mental health crisis, formulating evidence-based care plans.

• Deliver home-based crisis interventions, coordinate daily clinical clinic tasks, facilitate hospital step-downs, and support admissions to on-site hospitality beds to prevent crisis escalation.

• Provide clinical supervision and annual PDRs for junior staff, mentor learners, and collaborate with inpatient, primary care (Heeley PCN), and VCSE teams to refine neighborhood care pathways.

Requirements

• Professional qualification and active UK registration as a Registered Mental Health Nurse (RMN) with the NMC or as an AHP/Social Worker with Social Work England or HCPC.

• Current Band 6 standing or a minimum of 2 years post-qualification experience at Band 5 level, including community mental health and crisis/scheduled assessment experience.

• Valid UK driving licence with access to a vehicle for community visits.
