Position Overview
Role: Support Worker - Assisted Discharge
Location: West Middlesex University Hospital, London
Salary: £24,479 per annum, pro rata (plus Inner London Weighting pro rata)
Hours: 26 hours per week (December–March weekend shifts 10:00 AM – 6:00 PM as per rota)
Contract: Fixed-term contract until the end of March 2027 with the possibility of an extension
Closing Date: 23:59 on Tuesday, 6th October 2026

In this role, you will be a lifeline for individuals feeling vulnerable after a hospital stay. Whether providing practical help with shopping, checking in via phone calls, or simply being a friendly face, your support will help people recover safely and live more independently. No formal care experience is required—if you are patient, thoughtful, and compassionate, we want to hear from you!
Key Responsibilities
Practical & Emotional Support: Deliver welfare calls, assistance with shopping, and home visits to support recovery.
Hospital Discharge Care: Help adults transition smoothly and safely from hospital back to their own homes.
Referral Management: Respond to referrals initiated by clinicians and community health professionals.
Needs Assessment: Evaluate service user requirements and follow tailored, personalised support plans.
What We Are Looking For
Personal Qualities: A kind, calm, and confident approach coupled with fantastic interpersonal skills.
Working Style: The ability to operate seamlessly both independently and as part of a supportive team.
Essential Requirements: A full UK driving licence and willingness to drive service vehicles.
Core Skills: Basic IT literacy and a strong commitment to following established procedures. (Note: Unfortunately, we are unable to offer visa sponsorship for this role.)
