Community Nurse Case Manager (Adult Complex Care)

Location: Field-based across Southwark, Hounslow, and Basingstoke (with 1 day per week at Wimbledon Head Office)

Hours: Full-time (Monday – Friday, 9:00 AM – 5:00 PM)

Salary: £45,000 per annum + excellent benefits

We are currently recruiting a dedicated Adult Community Nurse Case Manager to manage complex care caseloads and support clients in their own homes across Southwark, Hounslow, and Basingstoke.

What You Will Do

• Manage a caseload of adult clients with complex clinical needs, providing face-to-face clinical assessments and developing personalized care plans.

• Deliver clinical training and mentor care teams, equipping support workers with the confidence and skills required to deliver high-quality home care.

• Collaborate closely with clients, families, multidisciplinary teams, and NHS Continuing Healthcare commissioners.

What We Are Looking For

• A Registered Nurse with an active NMC PIN and a minimum of 12 months of UK nursing experience.

• Essential experience in complex care settings, including enteral feeding experience (tracheostomy, ventilator, ITU/HDU, or spinal care experience is highly desirable!).

• Strong clinical assessment, care planning, and autonomous caseload management skills.

• Full UK driving licence preferred for community visits, and valid evidence of the right to work in the UK.
