Contract Type: Permanent, Full-Time (37.5 hours per week, working flexibly between 08:00 and 20:00, Monday to Sunday)

About the Role

We are partnering with New Victoria Hospital in Kingston upon Thames to find an organised and passionate Connected Care Lead on a permanent, full-time contract. As a Connected Care Lead through Supportive Bridge, you will play a pivotal role in delivering an exceptional patient experience across new Connected Care pathways. This exciting position supports the growth of Connected Care services across the wider Bupa network, acting as a central point of coordination between patients, consultants, clinical teams, and Bupa Insurance to ensure seamless, efficient care journeys.

Key Responsibilities

• Coordinate referrals, bookings, and patient pathways across multiple Bupa services from initial referral through to treatment.

• Act as a key liaison between patients, consultants, clinical teams, diagnostics services, and Bupa Insurance, proactively monitoring progression to resolve any pathway delays.

• Coordinate and administer up to three specialist Multi-Disciplinary Teams (MDTs)—organising meetings, preparing documentation, and accurately recording outcomes.

• Support pathway performance monitoring, service improvement initiatives, and business development projects.

• Build strong, collaborative relationships with internal and external stakeholders.

What We Are Looking For

• Essential experience in healthcare administration, patient coordination, pathway management, or MDT coordination, working alongside clinical and non-clinical teams.

• Excellent organisational, workload management, and communication skills with a high attention to detail and accuracy.

• Proficiency in Microsoft Office applications and patient administration systems, with the ability to work independently in a fast-paced environment.

• Desirable attributes include an understanding of private hospital operations, knowledge of CQC and healthcare regulatory standards, and experience with healthcare performance metrics or service improvement methodologies.
