Location: Allied Medical Practice / Church Surgery / Ship Street Surgery, Brighton (Primary locations include School House Surgery, BN1 7GF, and Church Surgery, BN2 4ES)

About the Role
We are looking for an experienced Practice Nurse to join the primary care team at Integrated Care 24 (IC24) across our Brighton practices. In this varied and rewarding role, you will use your full range of clinical skills to deliver high-quality, patient-centred care, driving prevention, early intervention, and long-term condition management to support the health and wellbeing of our local community.
Key Responsibilities
Provide comprehensive patient assessments, treatments, screenings, and health education in accordance with the NMC Code of Conduct.
Manage and monitor long-term chronic conditions, such as diabetes, respiratory disease, and cardiovascular disease, developing individual management plans in partnership with patients and GPs.
Deliver preventative healthcare services, including NHS Health Checks, cervical screening, immunizations, travel health advice, and smoking cessation support.
Carry out routine clinical procedures such as wound care, ear care, phlebotomy, ECGs, and blood pressure monitoring.
Maintain accurate clinical records and coding to support QOF and clinical governance standards, while recognizing and properly escalating any safeguarding concerns.
Collaborate closely within a multidisciplinary team of GPs, healthcare assistants, and practice staff to ensure coordinated, effective care.
Candidate Requirements
Qualifications: Registered Nurse with a current, valid UK NMC registration.
Experience: Previous clinical experience working within general practice or primary care, with proven capability in managing long-term conditions.
Skills & Attributes: Strong clinical judgement, safe decision-making skills, excellent communication abilities, and confidence working both independently and as part of a multidisciplinary team.
