About the Role
Supportive Bridge Staffing Agency is partnering with Bradford District Care NHS Foundation Trust to recruit motivated and compassionate Community Staff Nurses (Band 5) to join their dynamic locality teams.
This is a fantastic opportunity for both experienced and newly qualified nurses looking to deliver high-quality, person-centred care directly to patients in their own homes. You will play a vital role in helping individuals remain independent, safe, and well within their community, covering a diverse caseload ranging from long-term conditions to complex and palliative care.
Key Responsibilities
Deliver high standards of clinical nursing care within patients' homes and clinic settings as part of the district nursing team.
Collaborate closely with the wider Primary Health Care team to assess health needs and manage planned care.
Work flexibly on a rotational 7-day service rota (shift patterns fall between 8:00 AM and 9:00 PM).
Utilise agile mobile technology to ensure timely documentation and seamless communication with multidisciplinary specialists.
Essential Criteria
Registration: Active registration with the Nursing and Midwifery Council (NMC) as a Registered General Nurse (RGN).
Experience: Understanding or experience of interagency working and adult safeguarding.
Requirements: Full UK driving license and access to a vehicle for work purposes, as extensive travel across the North Bradford district is required.
