Supportive Bridge Staffing Agency is partnering with Bradford District Care NHS Foundation Trust to recruit an experienced and compassionate Specialist Public Health Nurse for Children in Care and Care Leavers (Band 6).
In this role, you will work as part of a district-wide health team delivering statutory health assessments and promoting strategies to improve health outcomes for vulnerable children and young people. You will manage a complex caseload, utilizing trauma-informed practices to empower young people to engage in their health care planning while collaborating closely with multi-disciplinary and partnership agencies.
Statutory Health Assessments: Provide comprehensive health assessments for children and young people under the care of the local authority.
Caseload Management: Oversee a complex caseload, demonstrating in-depth knowledge of trauma-informed care, Adverse Childhood Experiences (ACEs), and child protection issues.
Empowerment & Advocacy: Enable and empower young people to actively participate in assessing their health needs and developing personalized plans.
Multi-Agency Collaboration: Work effectively across agencies and alongside multi-disciplinary teams to achieve optimal health and wellbeing outcomes.
Professional Qualifications: RGN (1st level) or RMN qualification, combined with a Health Visitor certificate/diploma/degree or School Nurse experience/degree.
Experience: At least 2 years post-SCPHN qualification experience working with vulnerable children and families while holding a defined caseload.
Knowledge: Sound understanding of relevant children's legislation, child protection issues, and the health needs of Looked After Children.
Requirements: Appropriate UK professional registration and a commitment to maintaining the legal Right to Work in the UK.
