Salary: £28,557.79 - £32,802.41 Per Annum (CGL points 23 to 28)

Supportive Bridge Staffing Agency is proud to partner with Change Grow Live (CGL) to recruit a resilient, compassionate, and dedicated fixed-term Recovery Coordinator for their Intensive Support Team within the Severe Multiple Disadvantage (SMD) Programme in Nottinghamshire.
In this vital role, you will manage a specialized caseload of individuals facing severe multiple disadvantages and additional barriers to accessing services. Working across South Nottinghamshire, you will build trusting relationships, manage comprehensive care and risk plans, coordinate multi-agency case conferences, and deliver tailored substance misuse interventions to help clients achieve the best possible outcomes.
Intensive Caseload Management: Manage a targeted caseload accepted via the Intensive Support Panel, providing high-level engagement and wrap-around support packages.
Risk & Plan Management: Oversee service user support plans and risk assessments, using models like Motivational Interviewing to advise on substance and alcohol use.
Multi-Agency Coordination: Work closely with core teams, housing providers, and mental health agencies, convening and contributing to multi-agency case conferences.
Partnership & Advocacy: Actively represent the team at external forums, build robust pathways, and maintain accurate, confidential electronic case records.
Experience & Knowledge: Strong understanding of substance and alcohol misuse, mental health issues, and the complexities surrounding individuals with multiple and complex needs.
Multi-Agency Expertise: Experience facilitating multi-agency meetings and working collaboratively within a strength-based, asset-building approach.
Core Competencies: Excellent communication, negotiation, and advocacy skills, alongside high IT literacy and strong administrative capabilities.
Requirements: Patient, confident, and resilient approach capable of engaging clients who exhibit challenging behaviors or are hesitant to access services. Must hold a full driving licence, have access to a roadworthy vehicle, and possess the legal Right to Work in the UK (note: this role is subject to an enhanced DBS check).
