Care Navigator – Core Care Family Practice

We are looking for a friendly, compassionate, and organised Care Navigator to join the busy GP practice team at Core Care Links Ltd. In this essential patient-facing role, you will act as the first point of contact for patients, helping them navigate healthcare services and directing them to the most appropriate clinician or community service to ensure they receive the right care at the right time.
Act as the primary point of contact for patients via phone, face-to-face, and online channels.
Triage patient requests and effectively signpost individuals to appropriate NHS, primary care network (PCN), and community services.
Manage appointment booking, coordinate patient queries, and maintain accurate clinical records using practice systems (such as EMIS or SystmOne).
Work closely with GPs and the wider multidisciplinary team to support patient care pathways and practice demand management.
Ensure strict adherence to GDPR, confidentiality, and safeguarding guidelines at all times.
Education: Essential GCSE Maths and English at grades A–C (or equivalent); NVQ Level 3 is desirable.
Experience: Strong customer service, communication, and interpersonal skills. Previous experience in a GP practice, NHS, healthcare, or customer service setting is highly desirable, as is familiarity with clinical booking systems like SystmOne.
