Leeds Community Healthcare NHS Trust is recruiting a Band 5 Occupational Therapist for a rotational post. You will spend nine months at a time in different services run by Leeds health and care partners, covering areas such as long-term conditions, falls prevention, urgent response and hospital discharge. You will hold your own caseload, use health coaching to help people manage their conditions, and receive regular clinical supervision.

• Salary: £32,073 to £39,043 per year pro rata (Band 5)

• Permanent, 37.5 hours per week, with some weekend and unsocial hours

• Base: St George's Centre, Middleton, Leeds, moving with each rotation

• Must be a car driver with access to a car

• Interest in both physical and mental health
