Care Team Leader

In this key leadership role during the day shifts, you will guide and supervise care staff while working alongside Care Assistants to deliver exceptional care to residents. You will manage care planning, conduct risk assessments, oversee medication administration, and collaborate with multidisciplinary health teams and family members to ensure optimal resident outcomes.

Key Responsibilities

• Supervise and lead day shifts, delegate responsibilities, and work directly with care staff to support residents' daily living activities.

• Act as a case manager for residents, maintaining detailed care plans and coordinating with families and healthcare professionals.

• Administer medications safely, complete risk assessments, and partner with senior management to enhance service quality.

Requirements

• NVQ Level 3 in Health and Social Care (desirable) along with demonstrated experience supervising or leading a care team.

• Availability to work full-time day shifts, including alternate weekends.

• Well-organized, IT literate, adept at allocating work, and experienced in (or happy to undergo training for) medication administration.
