Supportive Bridge Staffing Agency is proud to partner with Change Grow Live (CGL) to recruit a dedicated, flexible, and compassionate permanent-track Young Person's Resilience Worker - Smoking Cessation to join their Kirklees team on a 12-month fixed-term contract.
In this vital role, you will act as a main care coordinator overseeing assigned non-opiate service users, focusing specifically on smoking cessation, harm reduction, and positive lifestyle changes. Working across home visits, community venues, and drop-ins, you will deliver trauma-informed 1-to-1 and group support, provide Very Brief Advice (VBA) training to staff and partners, and help vulnerable groups access mainstream community services.
Care Coordination & Support: Act as main care coordinator for assigned service users, providing 1:1, group, drop-in, and telephone support for individuals wishing to become smoke-free.
Smoking Cessation & Harm Reduction: Utilize a trauma-informed approach to promote positive lifestyle choices, raise awareness of the health impacts of smoking, and implement national stop-smoking strategies.
Training & Capacity Building: Deliver Very Brief Advice (VBA) training to CGL staff, volunteers, and local partner organizations (such as Emerging Futures and Kirklees in Recovery).
Multi-Agency Collaboration: Work within a multi-disciplinary framework to engage professionals, support community health education, and achieve service targets.
Experience & Knowledge: Strong understanding of substance misuse issues, health/social care environments, and national stop-smoking policies/strategies.
Intervention Skills: Experience working with service users presenting complex needs (including mental health, physical health, and criminal justice backgrounds).
Core Competencies: Excellent written and verbal communication skills, strong IT proficiency, and a proactive, self-motivated approach to time management.
Requirements: Supportive team player. Must hold the legal Right to Work in the UK (note: this role is subject to an enhanced DBS check).
Annual Leave: 25 days of annual leave plus bank holidays, increasing by 1 extra day for each year of service during the first five years (up to 30 days).
