Supportive Bridge Staffing Agency is proud to partner with Bradford District Care NHS Foundation Trust to recruit a compassionate and enthusiastic Care Coordinator (Band 5) for their South and West Community Mental Health Team for Older People (OPMH).
In this vital role, you will manage a dedicated caseload, providing holistic assessments, care coordination, and recovery-focused interventions. You will support older adults presenting with complex mental health needs, memory difficulties, reduced mobility, and social isolation, empowering them to maintain their independence and improve their overall quality of life.
Caseload Management: Act as Care Coordinator for a designated caseload, undertaking comprehensive mental health, risk, and care needs assessments.
Person-Centred Care: Develop, implement, and review recovery-focused care plans in collaboration with service users, carers, and multidisciplinary teams.
Clinical Support: Monitor, administer, and support compliance with prescribed medications in line with Trust policies and professional guidelines.
Service Development: Contribute to the ongoing development of the Memory Assessment Service and its pre-diagnostic assessment pathway.
Qualifications: Registered Mental Nursing Certificate (RMN) with active NMC registration.
Experience: Practical experience or clinical placement within a community mental health setting.
Skills & Knowledge: Ability to work autonomously, make sound clinical decisions, manage risks, and demonstrate strong communication, organisational, and interpersonal skills.
Requirements: Appropriate UK professional registration and a commitment to maintaining the legal Right to Work in the UK.
