Supportive Bridge Staffing Agency is proud to partner with Bradford District Care NHS Foundation Trust to recruit an experienced Community Mental Health Practitioner (Band 6) for their Community Mental Health Assessment Team.
In this integrated, multi-disciplinary team setting (working jointly with Bradford Metropolitan District Council), you will play a crucial role in delivering health and social care outcomes. You will be responsible for triaging and assessing routine referrals, managing a short-term care caseload, and providing high-standard, holistic mental health support to individuals and their families.
Triage & Assessment: Lead on routine referrals, signposting service users to appropriate agencies or the wider CMHT.
Caseload Management: Oversee a short-term caseload, conducting comprehensive risk assessments, care plans, crisis contingencies, and evaluations of care.
Multi-Agency Collaboration: Work closely with primary/secondary care professionals, housing, probation, police, and voluntary/third-sector agencies.
Leadership & Supervision: Supervise and mentor junior team members and student nurses, driving service development and clinical excellence.
Professional Registration: Current registration with the NMC (Mental Health Nursing), Social Work England (Social Work), or HCPC (Occupational Therapy).
Experience & Knowledge: Strong understanding of adult and children safeguarding, complex mental health/social issues, and mental health medication management.
Core Skills: Excellent verbal/written communication, effective workload prioritization, and the ability to work collaboratively in a multi-disciplinary environment.
