Job Summary

Supportive Bridge Limited is proud to recruit for a fabulous opportunity on behalf of the Derbyshire Healthcare NHS Foundation Trust. We are seeking an enthusiastic, motivated, and compassionate Band 6 Lead Nurse (RMN) to join the busy and dynamic Chesterfield Older Adult Community Mental Health Team (CMHT).

The CMHT supports individuals over the age of 65 experiencing both organic and functional mental illness. As a Lead Nurse, you will be clinically responsible for your own caseload, manage "duty" worker responsibilities, and collaborate within a robust multi-disciplinary team (MDT). This role offers an excellent platform to deliver high-quality patient assessments and interventions while actively progressing your leadership skills.

Main Duties & Responsibilities

Clinical Caseload Management: Support and manage a complex caseload of older adults with severe and enduring mental health needs.
Assessments & Care Planning: Complete comprehensive initial assessments, evaluate risk, and co-create collaborative, person-centered safety plans.
Duty & Interventions: Undertake duty worker responsibilities, administer depot injections, and actively participate in CST meetings.
Leadership & Mentorship: Supervise, support, and develop junior staff members and student nurses, fostering a positive and cohesive team culture.
Integrated Working: Establish strong collaborative relationships with internal teams and external health and social care partners.

Person Specification & Requirements

Qualifications & Professional Registration:

Must hold a current UK professional registration (RMN).
Degree or Diploma in Nursing.
Mentorship qualification (or equivalent) is highly desirable.

Experience & Knowledge:

Proven experience working with older people who have complex mental health needs.
Experience supervising junior staff and/or students.
In-depth understanding of relevant Mental Health legislation, safety planning principles, and CPA (Care Programme Approach) frameworks.
